Step-by-step help for filling out the application
Vereinsnummer: Filled by MTV
Vereinsname: MTV 1881 Ingolstadt
Letzter Verein: Enter if available
Passnummer: If known, otherwise leave blank
Name: Last name, First name
Geburt: Date of birth, Place of birth, Gender
Adresse: Street, House number, ZIP, City
Staatsangehörigkeit: For non-German nationality → see below
Spieler/in: Required from D-Youth onwards
Erziehungsberechtigter: Always required for minors
Verein: Signature & stamp by MTV
Page 2 describes cases where no waiting period applies when a child changes clubs.
For the application to be processed correctly, parents must provide appropriate documentation:
Proof: Confirmation that the player has not played any games for 6 months.
Without proof, the application will not be accepted.
Proof: Registration certificate from the registration office about the new main residence.
Application must be submitted within 3 months after moving.
Proof: Confirmation that the old club has withdrawn or dissolved the team.
For change to study location: Attach enrollment certificate.
For return after studies: Exmatriculation certificate.
Consent of new club required, provided no game has been played yet.

Always submit copy of ID document.
This page is only necessary if the deregistration from the old club is to be done online by the new club.

New club: Enter MTV 1881 Ingolstadt.
Previous club: Name of the releasing club.
Player signs.
For minors: also a legal guardian must sign.
This form does not replace the cancellation of membership in the old club (this must be done separately).
Club keeps the form for 2 years.
